Create Web UI to collect user data (name, email, company, etc.) or to opt-out when OpenNMS starts up after an install or upgrade. This will include the facility to save whether the user took an action (so it won’t be shown again), and to send the data to some endpoint (may not be fully integrated within the scope of this ticket).
