Mechanism to Store Permission to Collect Data - write to db or file, versioning of the agreement

Done Criteria:

• I can demonstrate that I can store agreements by version. (eg.  A file with a version number, or an entry in a db with a version number).
• I can demonstrate that I can store in the DB (or file) the agreement from the user.  (using SQL, or using a test script or program).
• I can demonstrate that a user is shown a different screen depending on the agreement status.
• (All the other Done, Done Criteria, testing ,review, doc, etc.)

Not Required:

• No screen asking for the agreement.
